This activity provides you with comprehensive trackers to record and monitor your health data over time.

The purpose is to create a clear picture of your current health status, track changes over time, and facilitate informed discussions with your healthcare provider.  This exercise lays the foundation for personalizing your weight loss journey and ensures you're addressing all aspects of your health.

Instructions:

  1. Download the Trackers below

  2. Familiarize yourself with the different sections:

    • Lab tracker (entries every 3 months)

    • Self-measure tracker (weekly entries)

    • Coming Soon Screening tracker (annual entries)

    • Coming Soon Medication tracker

    • Coming Soon Image/test tracker (entries every 6 months)

    Begin filling in the trackers with any information you currently have.

  3. Use these trackers during your healthcare appointments to record new data and discuss trends with your provider.

For reference, here are the laboratory tests and how I feel they should be categorized though every clinician is different.  Genetic information is excluded because we talk about it in later phases. Scroll down to review all the tests and categories.
